These terms define the conditions and procedures for using the Form8 service provided by COSTART Co., Ltd. (the "Company").

Members must comply with applicable laws and these terms while using the service, and must not infringe on the rights of others.

The Company may change features to improve service quality and will announce material changes in advance.

Form8 movement-quality analysis is non-medical wellness and sports-performance reference information. The Company does not provide diagnosis, treatment, injury prevention, medical judgment, or direct muscle-activation measurement from camera data alone.

If you have pain, a suspected injury, a medical condition, or discomfort during exercise, do not decide whether to continue exercise based only on the service result and prioritize professional judgment.
